General Information
I have read the program materials, program web pages, and Center for Talent Development Policies (CTD) web page, and fully understand the nature of the program, admission criteria, course cancellation, withdrawal, refund and financial aid policies. My child and I accept the terms and conditions and agree to abide by the policies stated in the available program materials. We will read the online program handbook and acceptance materials online. As a parent/guardian, I give permission for my child to participate in the program and any related activities.
I certify that all statements and materials presented in the application are correct and complete, and agree that the submission of inaccurate, incomplete or misleading statements or materials constitutes grounds for revocation of admission or dismissal from the program.
Academic Records
I authorize CTD to contact officials at the Qatar Foundation for information needed to register and support my child’s participation in the program. I authorize CTD and Qatar Foundation to share information regarding my child’s participation as-needed to facilitate my child’s experience and to evaluate the program.
Waiver, Release and Indemnification
I understand and hereby acknowledge that there are dangers and risks to which the student may be exposed by virtue of the student's participation in Northwestern University's Center for Talent Development program. I understand that Northwestern University does not require the student to participate in this activity, but I voluntarily choose for the student to do so despite the possible dangers and risks. I therefore voluntarily assume full responsibility for any risks of loss, property damage or personal injury, including death, that may be sustained by the student, or any loss or damage to property owned by us, as a result of the student's participation in Center for Talent Development, except to the extent caused by Northwestern University's negligence or willful misconduct. In consideration of, and in return for, any services, facilities or other benefits provided to us by Northwestern University in connection with this activity, I hereby release, waive, discharge and covenant not to sue Northwestern University (and its trustees, officers, employees and agents) from any and all liability, claims, demands, damages, actions or causes of action whatsoever arising out of or relating to any loss, damage or injury, including death, that may be sustained by the student, or to any property belonging to us, in connection with the student's participation in Center for Talent Development, unless caused by the negligence or willful misconduct of Northwestern University, its employees or agents. I further agree to indemnify and save and hold harmless Northwestern University (and its trustees, officers, employees and agents) and each of them from any loss, liability, damages or costs they may incur as a result of the student's participation in Center for Talent Development. I recognize that I may be giving up, among other things, certain limited rights to sue Northwestern University for injuries, damages or losses the student may incur by virtue of the student's participation in Center for Talent Development. I also understand and agree that this waiver binds our heirs, executors, administrators and assigns as well as ourselves.
Data Collection and Research
I understand that by participating in a CTD program, any data I supply with the application or that is generated as a result of participation in the program is collected in CTD's databases, which are stored on cloud-based storage services following industry standard security practices and with which Northwestern University has agreements governing the sharing of stored data with third parties. I understand that aggregated and deidentified information may be used for program evaluation, quality assessment, strategic planning, and research purposes. CTD will strive to maintain appropriate safeguards to protect the security, integrity, and privacy of students' personal data. Non-personally identifiable or deidentified student information may also be shared with third-party vendors for research or program related services. I understand that information available to me on MyCTD or within CTD’s instance of Schoology (e.g., CTD Online courses) proprietary in nature, and I agree to only share it with others in the CTD community.
CTD Technology Platforms
Northwestern University is based in Illinois in the United States and is governed by laws and regulations enacted by the State of Illinois and the U.S. federal government. CTD's agreements with the providers of its main end user technology platforms require compliance with the Federal Educational Rights and Privacy Act (FERPA) and the Children's Online Privacy Protection Act (COPPA.)
Some CTD programs involve live virtual meetings using web conferencing software. These meetings may be recorded by Northwestern University personnel and made available to other students enrolled in the same program who could not attend live sessions for later viewing. Class recordings may also be viewed by other CTD staff for quality assurance purposes. By enrolling in a CTD program and allowing your student to participate in a web conference hosted by CTD, you are indicating consent for your child's participation in the web conference to be recorded and used for these purposes. Northwestern University prohibits students from making their own recordings of class meetings.
By registering your child for a CTD program, you are indicating consent for CTD to make use of these technology platforms in the delivery of programs and services to you and your child and to share individually identifiable student data with the vendors of these platforms for the purpose of providing the service.
By registering your child for a CTD program, you are also indicating consent for CTD to allow your child to use other online tools and resources it selects for individual courses and programs within the limits of FERPA, COPPA, other applicable laws, and CTD and Northwestern University policies.
Schoology Privacy Policy & Terms of Use
Schoology is a learning management system used by many of CTD's programs to support the delivery of online courses, communicate with parents, provide access to supplemental online educational resources, and provide a managed environment for collaboration and resource sharing in many face-to-face CTD programs. By registering your child for a CTD program, you give permission to CTD to create Schoology accounts for you and your child.
Usage of Schoology by Students
If a student is under the age of 13, a parent or legal guardian must give consent for the child to use Schoology unless the CTD course or assessment is provided on behalf of a school or school district in which the child is enrolled. By registering your child for a CTD program, you indicate your consent for your child to use Schoology and Google Workspace for Education. Schoology collects limited personal information from minor students only where that student's school, district, or teacher has contracted with Schoology to collect personal information for the use and benefit of the learning environment. Usage of Google Workspace for Education by Students
Some CTD programs use Google software applications that are part of Google Workspace for Education. By registering your child for a CTD program, you indicate your consent for your child to use Google Workspace for Education. You also acknowledge that (a) your child’s data accessible by Google may include personally identifiable information from education records that may be subject to FERPA ("FERPA Records") and (b) to the extent that student data includes FERPA Records, Google will comply with FERPA. "FERPA" means the Family Educational Rights and Privacy Act (20 U.S.C. 1232g) and the Family Educational Rights and Privacy Act Regulations (34 CFR Part 99), as amended or otherwise modified from time to time.
Exceptions
Exceptions to stated privacy policies may be made in certain limited circumstances. By registering your child for a CTD program, you indicate understanding that information collected through technologies used by CTD may be shared with certain third parties if Northwestern University or its providers are compelled to share information by a valid legal warrant or subpoena; if information shared creates reasonable suspicion that a students is in imminent danger or poses an imminent threat to the safety of others; or if a student discloses information that CTD staff are obligated to report to child protective service agencies as required by Illinois’ Abused and Neglected Child Reporting Act (325 ILCS 5).
